This is basically the tester I use when I am testing LEDs or other Signals.

You just need 3 SPST, Momentary Contact Switches, a set of Resistors and LEDs, Plugs and a cable if you are going to use RJ11 type connectors.

Add a jumper to eliminate any or all of the current limiting resistors.
